Strange MicroSD behavior

I got a MicroSD, 8GB, Kingston, class 4. The card works fine in a LAVA phone. But apart from that it doesn't work on any other device. Not in smartphones or in laptops. When I insert the card into a smartphone, it prompts to format the card before I can use it. Then I try to format it but fails and again prompts to format the card before I can use it. 

 

Then I try to format on laptop. "windows can not format the disk" is the error message. Tried formatting with diskpart command but it fails too.

Also make sure the sd card isnt locked/write protected etc by that phone because it works in only that device.

Tried this one and HP USB storage format tool already.

Procedure:

1. delete all paritions on USB drive

2. then create a primary parition  on USB drive

3. then format  primary parition on USB drive that  to FAT

If you still fail, throw away the SD card and buy a new one (preferably a good brand)
